Outcome of Continuous Epidural Infusion Combined with Transforaminal Epidural Injection for the Treatment of Postherpetic Neuralgia

Abstract
Background: Postherpetic neuralgia (PHN) is the most common and serious morbidity following the herpes zoster infection in elderly patients. However, so far there is no satisfactory treatment option for relieving PHN and nerve blocks may not considered as an effective pain treatment.

Methods: Seventy-three patients of a total who were above 50 years old and suffered from PHN over 2 months were included in the study. They underwent continuous epidural infusion (0.3% mepivacaine 5 ml/hr) combined with transforaminal epidural injection (0.3% mepivacaine 3 ml + triamcinolone 10 mg). We compared pain score with visual analogue scale (VAS) and dose of analgesics in pre- and post-procedure 1, 2, and 6 months.

Results: There was a significant reduction of VAS in the post-procedure 1, 2, and 6 months compared to pre-procedural period. There was no difference in VAS among each post-procedural period. There was no change in doses of analgesics between the pre- and post-procedural periods.

Conclusions: Continuous epidural infusion with local anesthetics and transforaminal epidural injection with local anesthetics and low dose of steroid could be an effective treatment for pain relief in the PHN patients without serious complications.
